Our Medication Assistance Program
CHI’s Medication Assistance Program offers qualified patients prescription medications at extremely low reduced rates.
A patient must meet financial guidelines in order to qualify for the program. Most medications qualify, but some must also meet guidelines to qualify.
When a prescription is approved by the corresponding drug company for a reduced rate, it will be routed for pick-up by the patient to the CHI pharmacy of the patient’s choice. Or free home delivery can be arranged by notifying the pharmacy when you submit your prescription.
CHI’s Doris Ison, Martin Luther King Jr. Clinica Campesina and West Perrine health centers all have staff available to assist you with the application for this program.
