The CHOICE Program, started in 2024, connects patients living with diabetes to personalized support to help them better manage their condition.
For nearly two decades, Argentina Garcia struggled with unmanaged diabetes. Diagnosed 19 years ago, her A1C levels remained out of control until a routine visit with Dr. Elizabeth Philippe at Community Health of South Florida, Inc. (CHI) set her on a new path toward better health.
“I was very interested in the CHOICE program because of my focus on my health. I enjoy engaging with people and learning, which is simply who I am,” said Garcia. “I aim to find the positive in every situation, especially regarding my health and personal experience.”
Recognizing the need for additional support, Dr. Philippe referred Garcia to the CHOICE Program. It connects patients living with diabetes to education, resources, and personalized support to help them better manage their condition. CHOICE links patients to trusted community partners like the YMCA, Feeding South Florida, the American Heart Association and more, making sure patients have access to essentials like nutritious food, transportation, and wellness resources. The program is supported by the Health Foundation of South Florida and Health Choice Network.
Beyond clinical care, the CHOICE program promotes something equally powerful: connection. Patients are encouraged to engage with one another, creating a sense of community where individuals feel seen, supported, and understood. For many, including Garcia, this shared experience helps ease the emotional weight of managing a chronic condition while providing access to valuable health education and guidance from professionals.
Eunice Hines, Director of Migrant Services and Outreach at CHI, has witnessed the challenges patients face.
“Managing diabetes can be costly and overwhelming, especially for those without insurance, as patients often have to balance expenses like food, housing, and medication,” she said. “Education is key, understanding nutrition, how to manage the disease, and what happens when it goes untreated can make all the difference in preventing serious complications.”
Through collaborations with community agencies, the CHOICE program specifically supports patients with A1C levels over 9, helping address barriers that extend beyond the exam room. This includes housing and employment as well as transportation and access to healthy foods. Patients receive the support they need to take control of their health.
Today, Garcia’s diabetes is under control, and she credits the program with changing her life. Her journey stands as a powerful example of what’s possible when patients are given the tools, resources, and encouragement to succeed.
“Don’t be afraid; everything is possible,” Garcia said. “I managed uncontrollable diabetes with a very high A1C and tried all available programs to overcome challenges. The key is to practice what works for you, consult with your doctor and collaborate to find the best options, enabling us to learn from each other. I had a very positive experience with Dr. Philippe.”
As the program continues to grow, Hines hopes more patients will take advantage of its benefits.
“I would like to see many other patients with uncontrolled diabetes take advantage of this wonderful program that can only benefit them,” Hines said. “It addresses non-medical factors that influence positive health outcomes,” she said.
Through stories like Garcia’s, CHI’s CHOICE program prove that with the right support, education, and community, lasting change is not only possible but also happening every day.
