Registration is required. Space is limited, so apply soon. First reserved, first served.
Steps for registration:
- Scan the QR code to download the Resource Assistance Navigator app.
- Follow the instructions for eligibility determination by family size.
- Choose how you will receive your Farm Share items.
You may also see a CHI volunteer at the front desk of the Doris Ison Health Center to register.
